Health Services
The school nurse is available throughout the school day for student assistance in case of illness, injury, or other health related concerns. Students visiting the health office will be evaluated on an individual basis. Parents will be notified by phone if the student's injury or illness is of concern. Students visiting the nurse must have a pass from their teacher except, of course, in cases of emergency.
Medication at School
Batavia City School District has established the following policy when there is a need for a student to receive medication (both prescription and over the counter medications) in school:
-Medication must be in the pharmacy container or the original bottle.
-All prescription drugs and non-prescription drugs to be dispensed by the nurse in school require a doctor's order and written parent permission, on file, in the student's permanent health record. See pdf to download form at bottom of page.
-Students may carry medication and self-medicate as long as the following conditions have been met: a letter is on file from the doctor stating that the child is mature enough and responsible to do so; a letter is on file from a parent indicating the same, granting permission, and assuming responsibility for educating the child regarding responsible behavior; the student is not found to be irresponsible by school officials.
-Medication orders must be renewed annually or whenever there is a change in the original prescription. At the end of each school year, parents should pick up any unused medication. Medication not picked up by parents at the end of the school year will be discarded in early summer. MEDICATION FORM
Mandated Screening and Physicals
NYS law requires physical exams/health appraisals for all students who are new to the district and those students who are entering 9th or 11th grade. The exam can be done as early as one year prior to the date that the student enters 9th or 11th grade or entrance into our district. The health appraisal form is sent to all 8th and 10th graders with their June report card and can also be printed from this website. The health appraisal form must be submitted to the nurse's office by October 1st.
If the physical is not recived by that date it will be completed by the school practitioner or doctor. Physical exams are required for particpation in interscholastic sports and are valid for one year.

Body Mass Index
The NYS Education Law requires Body Mass Index (BMI) and Weight Status Groups will be included as a part of the student's mandated grade Health Appraisal. All information gathered is considered medically confidential. A sample of school districts will be chosen to take part in a survey by the NYS Health Department. Any sharing of data that will be required will be by age and gender only, not by name. Only summary information is sent. However you may choose to have your child's information excluded from the survey report. The information sent to the NYS Department of Health will help health officials develop programs that make it easier for children to be healthier.
Dental Exams
Also, included on the revised Health Appraisal is new documentation regarding dental exams. At this time, it is recommended that your child should be scheduled to a dental exam along with the mandated grade health appraisal.

Immunization Requirements
Immunization is required prior to registration in Batavia City School District. Failure to provide proper proof of immunization will result in your child’s inability to attend school on the first day. Please contact the school nurse for any questions.
All entering students must provide signed documentation from a physician or prior school health record showing that the following immunization requirements have been completed:
